The main objective of the Programme is to introduction of personal assistance services as a form of publicly available support in performing everyday activities and functioning in social life.

2. People with disabilities holding a certificate:

• with a significant degree of moderate disabilities
• disability,
• treated on an equal footing with the judgments referred to in points (a) and (b), in accordance with Articles 5 and 62 of the of 27 August 1997 on vocational and social rehabilitation and employment of persons with disabilities.

2. Personal Assistance Services consist in the assistant supporting a person with a disability in various spheres of life, including:

• in activities including maintaining personal hygiene;
• in performing daily activities related to functioning in the home and family;
• in moving outside the place of residence;
• in communicating with others including expressing their needs and understanding
• and the transmission of information;
• in participation in life social activities (e.g. undertaking social, educational, professional activities) cultural or cultural).

Interested people who want to to take advantage of the above-mentioned program, they are asked to fill in the application form, which is downloadable and can be found in the attachments below.

A completed Application Form, GDPR clause together with a certificate of disability must be provided to the headquarters of the Municipal Social Welfare Center in Ełk, Marsz Street. J. Piłsudskiego 8,
Monday to Friday from 8:00 a.m. to 3:00 p.m. or via public registered electronic service or electronic service Public Administration Services Platform.

At the same time, we would like to inform you that expressed by your willingness to participate in the "Personal Assistant of a Person with Disabilities" Program disability" for Local Government Units – 2027 edition, no is tantamount to qualifying for participation in the Program.

In case of receipt of funds financial instruments for the implementation of the Programme, an assessment of the individual situation of a person with a disability in accordance with Appendix No. 2 of the Evaluation Card individual situation of a person with a disability that will be fulfilled by employees appointed by the Implementer.

The assessment will be carried out on the basis of based on:

• application form for the Programme;
• documentation submitted by a person with disability (legal guardian);
documentation in possession of
• the implementer of the Programme;
• interviews with a person with a disability or his/her legal guardian;
• information obtained during the site visit residence of a person with a disability.

The municipality, by granting care services takes into account the needs of the person  with a disability requiring the support of an assistant, the level of independence of a person with a disability, the possibility of obtaining help from other people.

On this basis, and on the basis of the conditions set out in the Programme, we will select the people most in need of this type of support.
